The applicant or licensee shall submit all fees in this section that apply:

1. AIT program application

4. Verification of licensure requests from other states


5. Nursing home administrator license renewal


6. Preceptor renewal


7. Penalty for nursing home administrator late renewal


8. Penalty for preceptor late renewal


9. Nursing home administrator reinstatement


10. Preceptor reinstatement


11. Duplicate license


12. Duplicate wall certificates


13. Reinstatement after disciplinary action
